Report an Incident

Report an Incident is the guided way to file an incident — three short screens, built for the person who just saw something happen. You don't need to know any regulations, and only the first line is required. EHS staff can refine everything later from Incident Manager.

You'll find it two ways:

• the + Incident quick-create panel offers Report step-by-step beside the quick form, and
• Incident Manager's sidebar has Report (guided).

The three screens

1. What happened

One line about what happened, which plant it happened at, and who owns it. Those three are required — an incident belongs to a site, and one nobody holds is in nobody's list. Who owns this? opens on your workspace's EHS Principle, and any active person can be chosen instead.

Then as much of the story as you have: details, where exactly, and when it happened and was found. Leave the times blank if you're not sure — filing the report records when your organization learned of it, which is what the OSHA clock measures.

If the report turns out to be on an OSHA reporting timeline — a fatality within 8 hours, an amputation, in-patient hospitalization or loss of an eye within 24 — three things happen as it is filed, whoever filed it and whoever they picked:

• it is assigned to the EHS Principle, because those deadlines are legal ones and the Principle is the person answerable for them;
• it opens at Critical, the highest priority, so it is not sitting at Medium beside a near-miss while the clock runs;
• the Principle gets a message in evo.ehs and an email, straight away. Neither waits for them to be signed out, and neither can be muted — the ordinary rules for message notifications do not apply to a reporting deadline.

You do not have to know which timeline applies, or whether one does. The duties are worked out from what you answer on the second screen, so an incident earns its clock from what happened rather than from anyone recognising the rule.

2. Who was hurt, and how bad

Four plain questions: someone died, someone was admitted to a hospital, an amputation occurred, someone lost an eye. Tick anything that applies — if one of them creates an OSHA reporting duty, the wizard says so immediately, with the deadline.

Tick one and a second short list appears: the things OSHA does not want reported. A hospital stay that was tests or observation only rather than an admission for treatment; a vehicle accident on a public street or highway, unless it was in a construction work zone; anything on a commercial plane, train, subway or bus. Tick one of those and the wizard says so just as plainly — "This does not have to be reported to OSHA …" — with the reason, and a note that it may still belong on the OSHA 300 log.

The list stays hidden until one of the four outcomes is ticked. Reporting a near miss should not walk anyone through four questions about hospitals.

3. Review & create

A summary of what you entered, and — before anything is saved — the duty stated plainly: "This must be reported to OSHA within 24 hours — by 4:12 PM on Aug 30. Call 1-800-321-OSHA (6742) or report at osha.gov/report." Click Create incident report to file it.

After filing

Photos and files are added here, not on the first screen. Each one is saved to the incident the moment it uploads, so you can close the page, refresh, or be interrupted without losing it. There is nowhere to store a file before the report exists, which is why the step comes after filing rather than during.

The confirmation shows the new incident number and, when a duty applies, the live deadline banner with Mark reported to OSHA — the same flow described in Incident Manager. From there you can open the record in Incident Manager or report another.

The wizard uses the same rules as the full form: the outcomes you tick set the OSHA duty flags automatically, and everything you enter lands on a normal incident record — nothing about the guided path is a separate system.
